I know it's kinda late but you guys should really be using NukeSentinel.
PHPNuke is notorious for having a problem with SQL Injection. Basically, anyone with SQL knowledge can type a SQL statement into a form field, hit submit, and have the command excuted. What type of commands? Well, as some of you have found out they can change login names, passwords, etc.
NukeSentinel monitors and validates SQL requests made through the mainfile.php of PHPNuke. Anyone attempting to SQL inject will automatially have their IP banned and logged in PHPNuke. You also have the option to have the IP banned in the .htaccess file which prevents that IP from accessing any part of you site, not just PHPNuke.
There's also a block that will scroll a list of IP's your Sentinel has banned.
